At Central Ohio Endodontics, our goal is not only to provide you with the finest endodontic care, but offer you the most convenient financial options as well. We will work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. Please bring your insurance information to your first office visit.
If you have insurance, we will contact your insurance company for an estimate of your financial responsibility and collect that portion on the day of service. This is an ESTIMATE based on information provided by your insurance carrier. Upon completion of services, we will submit a claim to your insurance company. Most insurance companies will respond within four weeks. Any remaining balance after insurance has paid is your responsibility and we will send you a statement. Your prompt remittance is appreciated. Likewise, in case of overpayment, it will be reflected on the statement and we will promptly process a refund.
Patients should be aware of their insurance network and coverage in order to best utilize their benefits. You are encouraged to call your insurance provider directly.
Please remember that you are fully responsible for all fees charged by this office, regardless of your insurance coverage.

At Central Ohio Endodontics, we make every effort to provide you with the finest endodontic care and the most convenient financial options. For self pay patients (no dental Insurance coverage); we ask that payment in full is made at the time of treatment. For your convenience we accept Visa, Master Card, American Express and Discover.
Although we try to maintain a current list of plans we accept, please confirm coverage with your insurance carrier. We currently are in-network providers for: Aetna, Assurant, Careington, Delta, LHI, Liberty, MetLife and Superior. We are providers for additional plans through alternative networks such as Medical Mutual, United Concordia, DHA, and SunLife. Please call your insurance carrier to see if we are a preferred provider on your plan.
As a service to our patients, we also accept CareCredit. CareCredit lets you begin treatment immediately and provides the opportunity to make low monthly payments that fit into your budget. CareCredit offers a full range of No Interest and Extended Payment Plans to cover your treatment fees. Contact CareCredit for further details.

- Your referral slip and any X-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
- A parent or guardian must accompany all patients under 18 at the consultation visit.
- Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental treatment (i.e antibiotics for joint replacement or artificial heart valves).
- If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If additional films are necessary, they can be taken at our facility, which is included in the fee of consultation or treatment.
