The fee for endodontic therapy is determined by the complexity of the tooth being treated. When a microsurgical procedure is indicated initially or becomes evident during or after endodontic therapy, a separate fee may be charged. Consultation fees are payable at the time of consultation. Our endodontic therapy fee includes consultation, treatment plan, clinical procedures, and follow-up care, excluding the final restoration completed by your family dentist.

It is the policy of this office that fees for treatment be paid as follows:

1. Self pay patients (no dental insurance coverage); payment in full requested at time of treatment with cash, check, Visa, MasterCard, or Discover
   
   
2. Patients with primary insurance coverage, 30% payment of total amount is due at completion of treatment with cash, check, Visa, MasterCard, or Discover
   
   
3. Patients with Delta Dental insurance coverage, 20% payment of total amount is due at completion of treatment with cash, check, Visa, MasterCard, or Discover
   
   
4. Patients with both a primary and secondary insurance plans will not be required to make a payment at completion of treatment. Claims will first be submitted.

All insurance payments are assigned to our office unless your account is paid in full at time services are rendered. Please bring your insurance information with you to the consultation so that we can expedite reimbursement. We are not providers of Delta Dental of Ohio, Indiana, Kentucky. If necessary, special payment options may be provided to our patients. Please contact us for specific questions regarding payment prior to your appointment.
